Privacy Policy

At  Awaken, we are committed to protecting your privacy and the confidentiality of your Protected Health Information (PHI). Under the Privacy Act, you have several important rights regarding your PHI:

1. Request Restrictions: You can request restrictions on how we use or disclose your PHI for treatment, payment, or health care operations. Submit your request in writing to our Privacy Officer, specifying the information to restrict, the type of limitation, and to whom it applies. While we are not required to agree, we will comply if we do, except in emergencies.

2. Communication Preferences: You can request specific methods or locations for receiving communications that include your PHI. Submit your request in writing to our Privacy Officer. Texting is not secure; use our HIPAA-compliant email for all confidential communications.

3. Access to PHI: You have the right to inspect and obtain a copy of your PHI in our designated record set, excluding psychotherapy notes. Submit your request in writing to our Privacy Officer. We may charge a fee for copying, mailing, or supplies. We will respond within 30 days or inform you of any delays. If your request is denied, you can request a review by another licensed health care professional.
